Open Records Request
You must complete and submit a Request for Records Form; prepayment for the records may be required. If an insurance company or attorney wishes to request an accident report, requests must be submitted in writing on letterhead together with a completed DPPA Limited Vehicle/Driver Record Information Request Form.

Q. How long do requests take and how much do they cost?
A. All requests are fulfilled on a first come first served basis. Responses to a request are completed as soon as practicable and without delay; however, larger, more complex requests will take longer to be completed. The cost is $0.25 per page and $3.00 per disc (photos, audio or video recordings). Prepayment may be required if the total fee exceeds $5.00. Postage fees will be added when records are mailed. Electronic (video) records will take longer to process.
Q. What is no longer generally available?
A. Under 18 U.S.C. § 2721(C), the following information about a person is NOT available to the public:
- Name
- Address
- Date of Birth
- DMV Photograph
- Social Security Number
- Telephone Number
- Medical or Disability Information
- Driver's License Numbers and Physical Descriptors
- Vehicle Registration Plate Numbers and Related Information
- Vehicle Identification Numbers (VIN)
Q. How can I get personal information?
A. The Ashwaubenon Public Safety Department Request for Records with DPPA form provides you with the opportunity to let us know of the DPPA Exemption/s under which you are requesting personally identifiable information regarding your own or another person's record.
- With or without a qualifying exemption, the record is subject to release; however, personally identifiable information may not be included.
